We are looking for an experienced compassionate and motivated Case Manager to join our Eastwood office on a full-time basis. In this role, you will work closely with clients receiving Support at Home benefit, ensuring they receive quality, person-centred services that help them live independently at home.
What you'll do:
* Manage a portfolio of Support At Home clients.
* Develop, review and monitor care plans to meet client goals.
* Provide advice and guidance to clients and their families.
* Coordinate the delivery of all services by Support Staff, including scheduling/rostering
* Monitor package budgets and ensure compliance.
* Work Health and Safety issues/Risk Management
* A mixture of being in the office and being out in the community visiting clients
What you’ll bring:
* Qualification in Nursing, Community or Human Services, Case Management, Psychology, Social Work, Allied Health or Rehabilitation Case Management.
* Experience in case management and aged care.
* Strong knowledge of SaH funding and Aged Care Quality Standards.
* Great communication and problem-solving skills.
* Excellent documentation and computer skills
* Police Check / NDIS Worker Screening Check (or willing to obtain).
* Unrestricted current Australian driver licence and reliable car.
