工作介绍

ABOUT THE ROLE
Reporting directly to the Care partner team leadder, you will provide effective and high-quality care coordination and management services to support individuals living in their homes. Specifically, you will be responsible for facilitating care solutions for individuals and their families to ensure consistently high-quality care services to our home care clients. You will be supported by our local nursing team and health professionals.
You will be responsible for:
• Provide in-home assessments, referrals, care plan reviews & goal planning for consumers
• Client budget management & reporting
• Excellent time management with client caseload
• Respond & communicate effectively with clients re: changing care needs
• Relationship management with internal & external stakeholders
• Possess empathy & understanding towards the needs of older people

ABOUT YOU
You are a supportive, friendly, and organised individual with excellent time management, and customer services skills. You thrive in a dynamic and innovative environment and enjoy being part of a highly skilled team who believe in the care and service that they provide.

To be successful in this role you will have the following skills and qualifications:

SKILLS AND EXPERIENCES
• Bilingual and able to speak English and Mandarin is essential
• At least 2 year’s experience in aged care and Case Management
• Demonstrated understanding of relevant Commonwealth legislation as it applies to Support At Home and the Aged Care Quality Standards.
• Ability to develop and maintain excellent working relationships and demonstrated customer service skills.
• Knowledge of budget management and fees and costs associated with Support At Home
• Understanding of the interface between CHSP and SAH
• Ability to work independently and be a resilient problem solver
• Demonstrated strong awareness of risk and solid report writing and administrative skills.
• Proficient with client management systems, Microsoft office and government reporting funding portals
• First Aid Certificate
• Hold a current valid driver's license.
• A clear national police check upon role offer
